What is Children's Long-Term Support (CLTS)? 

The Family Support Team Service Coordinators utilize this federal and state program to fund for services in the child's home and community based on the child's diagnosed conditions. The purpose of this voluntary program is to help families support a child, with additional needs relating to a disability, in their home. Children served by this program may have needs relating to cognitive development, physical development, or social emotional development. The goal of this program is to support children in their home and community instead of at an institution.

What is Children's Community Options Program (CCOP)?

The purpose of this voluntary program is to help families support a child with additional needs relating to a disability, in their home. CCOP can fund supports and services that are not available through other insurance, Medicaid, or the CLTS program. Funding is limited in Winnebago County for this program.

CLTS or CCOP Eligibility

These voluntary programs are available to youth who meet the following criteria: 

  • Under the age of 22
  • Have a diagnosed condition
  • Reside in Wisconsin
  • Live at home or in another eligible community-based setting
  • Eligible for a Medicaid type
  • Meets an eligible level of care on the Long Term Care Functional Screen

CLTS Provider Registry: 

For any service CLTS would fund, it is a requirement for the CLTS program that providers are listed on the CLTS Provider Registry and accept payments from the 3rd Party Payer for CLTS:
