Behavioral Health Services Division

MISSION STATEMENT

To develop a comprehensive range of services offering continuity of care for persons with substance use disorder and/or mental illness. These services shall focus upon prevention, community based treatment and shall strive to enhance the individual’s independence and recovery. Services are provided based on individual needs, utilizing the most normalized, cost efficient and least restrictive settings whenever possible.

RESIDENCY REQUIREMENTS

To receive services through Winnebago County Department of Human Services Behavioral Health Clinic or Programs, you must be a resident of Winnebago County.  Funding for our programs is reserved for our residents and therefore must be utilized for such.
Crisis services are excluded from this residency criteria.

In order to assist our staff in determining the residency, we will require proof of residency.  The following documents, which must include the name of the person requesting services and a Winnebago County address, will be considered acceptable proof of residence documents for verification by reception staff.

  • Utility bill issued within the last 90 days
  • Bank, credit union, credit card, or mortgage statement.
  • Current and valid WI driver's license or State ID card.
  • Paycheck/stub.
  • Current lease.

Clients not in possession of these documents will need to have further screening by our intake specialist, assessor, or supervisor.
