What is Children's Long Term Support
The Children’s Long-Term Support (CLTS) Program is a Medicaid funded program for youth under the age of 22 who have a diagnosed condition, i.e., developmental disability, physical disability, and/or mental health diagnosis, reside in a home or foster care setting in Wisconsin, and meet functional eligibility through the screening tool.
Through a partnership between the Wisconsin Department of Health Services (DHS) and county human services departments, children and families in the CLTS Program have access to a range of supports and services to help children with disabilities and their families grow and live their best lives in their home and community.
