OWI & Intoxicated Driver Assessments

If you have been court ordered to have an assessment, contact 920-236-4734 to schedule an appointment between the hours of 8:00 a.m. and 3:30 p.m. Monday through Friday. *Please note that payment of $300 and signed fee agreement is due BEFORE scheduling and may be done in person, over the phone, or via DocuSign. 

Payment by cash, personal check, money order, or some major credit card is required before you will be scheduled for the assessment. Personal checks returned for any reason will require an additional $20.00 charge. Further, you will face being reported to the Department of Transportation as being noncompliant for failure to pay the assessment cost and your driving privileges will be adversely affected. Please bring your health insurance card with you in the event that you need a referral to an outside provider.

Do not drink alcohol or use any illegal use of drugs/substances 72 hours prior to your appointment. If alcohol or drug use is suspected, the assessment will be cancelled and a rescheduling fee of $150.00 will be assessed.

Note: It is the policy of the Winnebago County Department of Human Services to give priority admission and treatment services to pregnant women.
