Every successful transition starts with a plan. This involves planning for things like employment, independent living, and post-secondary education. For people with a disability, this can be an overwhelming process.

ADRC Is Here To Help!

The Aging and Disability Resource Center (ADRC) of Winnebago County is here to help you and your family with the transition from high school to the adult world.

Whether you plan to remain in school, or need help with getting a job, or are not sure what level of assistance you may need, the ADRC can help with next steps, FREE OF CHARGE. 

 

We work with anyone 17.5 years or older! Just call us – 877-886-2372. We’ll get you on the right path.

What is WCCoT?

The Winnebago County Community on Transition (WCCoT) is a regional community that comes together (networks) regularly to identify issues, barriers, supports and solutions to assisting youth and young adults with disabilities make a successful transition based on their post-secondary goals related to training or education, employment and independent living.
