Requesting Your COVID Test Results

For the general public, COVID testing results are obtained by contacting your physician. 


If you attended the Fox Cities Community Testing Site hosted by the National Guard (May 2020) you should be receiving a phone call from the National Guard when the results are available.

If it has been over 5 days since you were testing and you live in Winnebago County, you may request your results via this form and we will attempt to contact you within 24 hours.  

You will need to provide:
  • Name
  • Date of Birth
  • Address
  • Phone Number
  • Email

If requiring written lab results, you will also need to print and legibly complete a Release of Health Information (PDF)*


*If you require written lab results please expect longer delays based on business hours and your ability to submit required HIPAA documentation. Results can be delived via a phone call from public health staff in less than 24 hours. Both of these options are condition on the availability of your results in WEDSS (we do not have control over when this is accomplished).
