A

AAA – Area Agency of Aging; sub state planning and coordinating agency authorized by federal Older Americans Act; 6 in Wisconsin.

ADA – Americans with Disabilities Act; PL 101-336, as amended; 42 USC 12101-12213.

ADP – Average Daily Population

ADRC – Aging and Disability Resource Center; part of Family Care. The first place to go with your aging and disability questions. ADRC’s are service centers that provide a place for the public to get accurate, unbiased information on all aspects of life related to aging or living with a disability.

AE – Adults and Elderly

AFH – Adult Family Home (a); A home certified by a county social services or human services department, in which 1 or 2 adults unrelated to the operator reside and which provides a structured living arrangement for residents.

AFH – Adult Family Home (b); A home for 3 to 4 developmentally disabled adults who receive care and maintenance above the level of board and room, but not nursing care, from the person whose home it is, and who for the most part lived in the home when they were younger and it was regulated as a foster home for children. Certified under chp. HSS 82 rules.

AFH – Adult Family Home (c); A home for 3 or 4 adults who receive care, treatment and services above the level of board and room, but not nursing care. Licensed under chp. HSS 88 rules.

AFCSP – Alzheimer Family Caregiver Support Program

AFDC – Aid to Families with Dependent Children; former income maintenance program providing payments to low-income households with children. Replaced by W-2.

AIDS – Acquired Immunodeficiency Syndrome

ALZ – Alzheimer’s Disease

ANEW – Association of National Eligibility Workers

AOA – Administration on Aging in HHS; responsible at federal level for Older Americans Act programs.

APPEAL – A request for review of an action.

APS – Adult Protective Services

ASSISTED LIVING FACILITY – Popular name for a place where 5 or more adults reside which consists of independent apartments and which provides each tenant with up to 28 hours of supportive, personal and nursing services per week; 1997 Wisconsin Act 13 amended statutes to change official name to residential care apartment complex.

B-3 – Birth to 3

B-3 INITIATIVE – State funding for B-3 programs/services.

BADR – Bureau of Aging and Disability Resources

BADGERCARE PLUS – Wisconsin’s family health plan for children and parents in families with income below 200% of the federal poverty level and no health insurance.

BADGERCARE PLUS FOR CHILDLESS ADULTS – Basic healthcare to low income adults who do not have health insurance or have minor children. They do not need to be disabled. Applications processed by state.

BALTCR – Bureau on Aging and Long-Term Care Resources; in the DHS Division of Long Term Care.

BCA – Basic County Allocation; that part of a county’s allocation from the total funds appropriated under s. 20.435 (7)(b), (I) and (o), Stats., for community aids that can be used broadly y for social services and services for mentally disabled persons; administered by DCFS.

BCMH – Bureau of Community Mental Health; in the DHS Division of Long Term Care.

BDDS – Bureau of Developmental Disabilities Services; in the DHS Division of Long Term Care.

BFS – Bureau of Fiscal Services; in the DHS Division of Management and Technology.

BHCF – Bureau of Health Care Financing; in the DHS Division of Health; administers the Medical Assistance program.

BHMA – Bureau of Health Management and Analysis; in the DHS Division of Health.

BID – Background Information Disclosure

BMHSAS – Bureau of Mental Health and Substance Use Disorder Services; in the DHS Division of Long Term Care; Substance Use Disorder prevention and treatment.

BIRTH TO 3 – A program of early intervention services for children from birth through age 2 with disabilities, and for their families. Administered by county administrative agencies in accordance with ch. HFS 90 rules.

BLOCK GRANTS – Federal grants of aid to states; the 6 annual block grants to DHFS are Social Services, Community Services, Maternal and Child Health, Preventive Health and Health Services, Substance Abuse, and Mental Health; Department develops proposals for use of grants and obtains comments of public. Legislature holds hearings, and the final applications are submitted to the Federal government for the next year’s funding.

BOALTC – Board on Aging and Long Term Care; an independent state agency that advocates on behalf of elderly and disabled persons who are receiving long-term residential care, mainly by monitoring development and implementation of policies and program and investigating complaints about care.

BPH – Bureau of Public Health; in the DHS Division of Health.

BQA – Bureau of Quality Assurance; in the DHS Division of Long Term Care; responsible for licensing nursing homes, home health agencies, CBRF’s, hospices, approving hospitals, certifying health care providers for Medicare, certifying programs that train and test nurse assistants and aides and operating a registry of qualified nurse assistants and aides. Formerly known as BQC when part of DOH.

BS – Benefit Specialist; A person in each county, usually in the county aging office, who helps elderly persons obtain and keep Medicare, Medical Assistance and other human services program benefits.

BSD – Bureau for Sensory Disabilities; in the DHS Division of Long Term Care.

C

CADET – a correctional system program which is a boot camp for delinquent youth.

CAN – Child Abuse and Neglect

CAREGIVER MISCONDUCT REGISTRY – The Department’s record of persons who, while working as caregivers in specified department-regulated facilities or for specified department-regulated programs, were found to have abused or neglected a client or misappropriated a client’s property.

CARES – Client Access to Re-employment and Economic Support, the State's computer system used to determine eligibility for W-2, Food Share, childcare and Medicaid programs.

CARS – Community Aids Reporting System.

CBMAC – Community Based Medicaid Administrative Claiming; a program whereby states are eligible to claim federal Medicaid funds/reimbursement for qualifying Medicaid administrative activities performed by these entities.

CBRF – Community-based residential facility; a place in which 5 or more unrelated adults live and where they receive care, treatment, or services, but not nursing care on any permanent basis, in addition to board and room. CBRF’s are licensed by DHS under ch HFS 83.

CCDE – Center for Career Development and Employability.

CCR – Community Care Resources; a private foster home agency.

CCS – Comprehensive Community Services; a new Medicaid (MA) benefit for individuals with a major mental illness.

CESA – Cooperative Educational Services Agency.

CHAP – Community Housing Alternatives Program; provides Wisconsin Housing and Economic Development Authority financing (generally, revenue bond authority) to developers of housing for low-income elderly and chronically disabled persons.

CHAPTER 46 – of the Wisconsin Statutes; basic statute for the Department of Health and Family Services.

CHAPTER 48 – of the Wisconsin Statutes: called the Children’s Code, this collection of statutes covers court and agency responsibilities for children found abused, neglected, in need of adoption, or otherwise in need of protection and services, or who live in foster family or group homes, child-caring institutions, or shelter care or secure-detention facilities, or who attend day care centers.

CHAPTER 49 – of the Wisconsin Statutes: public assistance programs.

CHAPTER 50 – of the Wisconsin Statutes: nursing home, CBRF, hospital and hospice regulation.

CHAPTER 51 – of the Wisconsin Statutes: commitment for treatment of mental illness; and services for persons who are mentally ill, developmentally disabled, substance use disorder.

CHAPTER 54 – of the Wisconsin Statutes: pertaining to guardianships and protective placements.

CHAPTER 55 – of the Wisconsin Statutes: protective services for adults, and for developmentally disabled youths age 14 to 17, including court-ordered protective placements.

CHAPTER 252 – of the Wisconsin Statutes: prevention and control of communicable diseases.

CHAPTER 254 – of the Wisconsin Statutes; environmental health.

CHAPTER 938 – of the Wisconsin Statutes: called the Juvenile Justice Code.

CHAPTER 971 – of the Wisconsin Statutes; includes criminal commitments of the mentally ill. A criminal commitment to a mental health facility is for the purpose of examining a defendant with respect to competency to proceed, treatment directed at regaining competency, or for treatment after a finding of not guilty due to mental disease or defect.

CHAPTER 980 – of the Wisconsin Statutes; civil commitments of sexually violent persons.

CHILD CARE ASSISTANCE – Help for families below 185% FPL to pay for daycare expenses.

CHILD-PLACING AGENCY – A private agency that places children in substitute care. Licensed by DHS under ch. HSS 54 rules.

CHILDREN’S CODE – Chapter 48, Wisconsin Statutes.

CHIPS – Child in need of protection or services.

CHSD – County human services department organized under s.46.23, Wisconsin Statutes.

CHSP – Congregate Housing Services Program.

CLTS – Children’s Long Term Support Waiver; There is a waiver for children with a developmental disability, severely emotionally disturbed and/or physical disability.

CLTSFS – Children’s long term support functional screen.

CMI – Chronically mentally ill, in reference to an adult.

CMO – Care Management Organization; Entity that provides or arranges for services in the Family Care benefit. Each CMO develops a provider network to provide services to Family Care recipients who live in their own homes, nursing facilities, or other group living situations. The CMO must coordinate care across different delivery systems (including primary health care, Long-Term Care [LTC], and social services) and funding sources (including Medicaid fee-for-service and other commercial health insurance, Medicare, and funding sources for vocational and social services.

COALITION FOR ADVOCACY – A Madison-based organization that advocates on behalf of developmentally and physically disabled persons.

COM LIV – Community Living.

COMMUNITY LIVING ARRANGEMENTS – A generic name for community-based residential facilities for adults (CBRF’s), group homes for children, and private child care institutions, for purposes of s. 46.03(22), Wisconsin Statutes, dealing with the siting of these facilities and overriding of local zoning ordinances.

COMMUNITY OPTIONS PROGRAM – See COP.

COP – Community Options Program; a county-administered program to reduce the number of persons admitted to nursing homes; involves assessing the need of MA-eligible persons faced with nursing home placement or resident in nursing homes for that placement and doing case planning and case management and paying for gap-filling supportive services to make continued or new community residence possible.

COST SHARE (Spend down) – Sometimes members are required to pay a share of the cost of the services they receive.

COUNTY DEPARTMENT OF COMMUNITY PROGRAMS – The county agency established under s. 51.42, Wisconsin Statutes.

COUNTY DEPARTMENT OF DEVELOPMENTAL DISABILITIES SERVICES – The county agency established under s. 51.437, Wisconsin Statutes.

CPCS – Child Protection Children Services.

CPS – Child Protective Services.

CR – Community Reinvestment.

CRC - Civil Rights Compliance

CRO – Client Rights Office in the DHS Division of Mental Health and Substance Abuse Services.

CSBG – Community Services Block Grant.

CSDRB – Community Service Deficit Reduction Benefit; a program whereby completed certification forms and cost reports developed by DHS must be submitted. Costs reported by participating agencies must be based on allowable cost and cost-finding principles as instructed by the DHS. These costs are evaluated by the DHS for reasonableness and accuracy. Benefits are paid retroactively based on information provided in the cost reports, subject to DHS review and limitations.

CSP – Community support program; in communities, a program that coordinates care and treatment services provided to persons with severe and persistent mental illness. Regulated under HFS 63 rules.

CTS – Caretaker Supplement; a check provided to children of SSI recipients; administered by the Economic Support Division LTS team.

CTST EVALUATION – Competency-to-stand-trial evaluation; the assessment by a psychiatrist or psychologist of a person charged with a criminal offense who because of a mental disorder may be unable to understand the proceedings or help with his or her defense.

CWC – Central Wisconsin Center, a DHS residential facility in Madison for developmentally disabled persons.

CWW – CARES Worker Web; a web based system that is gradually replacing CARES.

D

DBS - Disability Benefit Specialist

DCFS – DHS Division of Children and Family Services.

DCS – Former Division of Community Services in DHSS.

DD – As a noun developmental disability or disabilities; as an adjective or adverb, developmentally disabled.

DDB – Disability Determination Bureau in the DHS Division of Health.

DDES – Division of Disability and Elder Services, formerly DHS Division of Supported Living.

DETOXIFICATION – Emergency treatment for the acutely intoxicated person. Can be medical or non-medical.

DEV DIS – Developmental Disability – A disability of a person manifested before the age of 22, and expected to continue indefinitely, attributable to mental retardation, cerebral palsy, epilepsy, autism, brain injury or another neurological condition closely related to mental retardation or requiring treatment similar to that required for mental retardation, and that results in substantial functional limitations in three or more major areas of live activity.

DHS (State of Wisconsin) – Department of Health Services; State agency that contracts with the MCO for Family Care and Partnership services.

DHS (Winnebago County) – Department of Human Services

DIRECT SERVICES – Services provided directly to people by agency staff rather than purchased by the agency from an outside provider. DCFS regional adoption and permanency planning staff are often referred to as direct service staff.

DJC – Division of Juvenile Corrections in DOC; formerly DYS in DHSS.

DME – Durable Medical Equipment; Covered by the Family Care benefit and includes items such as wheelchairs, canes, etc.

DMS – Disposable Medical Equipment; A benefit included in the Family Care program that supplies members with disposable medical supplies intended for one-time or temporary use, such as cotton balls, dressing materials, etc.

DOA – Wisconsin State Department of Administration.

DOC – Wisconsin State Department of Corrections.

DOH – DHS Division of Health.

DRGs – Diagnosis-Related Groups; specific classifications of illnesses into which hospital inpatients are grouped. Under Medicare and MA, hospitals are reimbursed a fixed amount, determined in advance, for each patient admitted for an illness in a given classification.

DRUG COURT – Currently a county sponsored program model designed to provide alcohol and/or drug treatment to non-violent offenders in an attempt to break the cycle of criminal behavior, alcohol and drug use and incarceration.

DSM-IV – The 4th edition (1994) of the Diagnostic and Statistical Manual of Mental Disorders published by the American Psychiatric Association.

DVR – Former Division of Vocational Rehabilitation in DHSS.

DWD – Wisconsin State Department of Workforce Development.

DWS – Department of Workforce Solutions, the DWD office that oversees the W-2 and other TANF programs.

DYS – Former Division of Youth Services in DHSS.

EAN – Elder Abuse and Neglect

EAP – Employee Assistance Program; in places of employment, identification and referral for counseling of persons who exhibit alcohol abuse, drub abuse, or emotional problems which affect their work.

EBD – Elderly, blind, disabled; a term referring to this type of Medicaid programs.

EBS - Elderly Benefit Specialist

EBT – Electronic Benefits Transfer; a system for delivery of food stamp benefits through use of a “swipe card” as an alternative to issuing Food Share coupons.

EDI - Electronic Data Interchange

EDS – Electronic Data Systems; the firm which currently serves as fiscal agent (the payer of claims) for Wisconsin’s Medical Assistance program and processor of Senior Care applications.

EFT - Electronic Funds Transfer

EIS – Early Intervention Services

EM – Electronic Monitoring

EMERGENCY ASSISTANCE – Help for families with minor children to obtain or maintain permanent housing.

EPSDT – Early and periodic Screening, Diagnosis and Treatment program for children from low-income families; financed by Title XIX; called Health Check in Wisconsin.

ES – Elderly Services within the Long Term Support Division and/or within the Economic Support Division.

ESD – Economic Support Division of the Department of Human Services; made up of the Family and ES teams. Staff determine eligibility for W-2, Childcare assistance, Medicaid, Food Share and some other public assistance programs.

ESL – English as a Second Language

ESS – Economic Support Specialist; term referring to eligibility workers for Food Stamp and Medicaid programs.

eWiSACWIS – Wisconsin Statewide Automated Child Welfare Information System.

F

FAIR HEARING – A hearing held by an Administrative Law Judge who works for the Division of Hearings and Appeals. This Division is independent of both the MCO and DHS.

FAMILY CARE – A comprehensive and flexible long-term care service system, Family Care strives to foster people’s independence and quality of life, while recognizing the need for interdependence and support.

FAMILY CARE PARTNERSHIP (PARTNERSHIP) – Formerly known as Wisconsin Partnership Program, integrates health and long-term support services.

FAMILY DAY CARE CENTER – A place where a person other than a parent, relative or guardian provides care and supervision for 4 to 8 children under 7 years of age for less than 24 hours a day, for compensation. Licensed under ch. HSS 45 rules.

FAS – Fetal alcohol syndrome; physical and/or mental abnormalities in a newborn child resulting from the mother’s drinking of alcoholic beverages during pregnancy.

FC – Family Care; A voluntary long-term care managed care program. The State contracts with Care Management Organizations (CMOs) that provide or arrange for services in the Family Care benefit. Each CMO develops a provider network to provide services to Family Care recipients who live in their own homes, nursing facilities, or other group living situations. Family Care will foster recipients’ independence and quality of life, while recognizing the need for support to remain independent.

FCCC – Fox Cities Community clinic; clinic for low-income people who don’t qualify for Medicaid. Supported in part by a grant from the Winnebago County Department of Human Services.

FDD – Facility for the developmentally disabled; A type of nursing home primarily for developmentally disabled persons. State centers for developmentally disabled persons are FDDs. Licensed under ch. HFS 134 rules.

FDL – Fond du Lac

FEP – Financial Employment Planner; term given to Economic Support Division ESS who works with W-2 participants and child care participants.

FFA – Foster Family Assessment; a very thorough assessment of people applying to be child foster parents, including their history and their current level of functioning.

FFES – Functional and Financial Eligibility Screen; A tool developed by DHS and used by trained Resource Center staff to determine functional and financial eligibility for Family Care.

FFP – Federal financial participation, i.e., Federal aid.

FISC – Financial Information Service Center; an agency which helps with budget counseling.

FNS – Food and Nutrition Service; the part of the U.S. Department of Agriculture responsible for Food Stamps.

FORWARD CARD – Medical Assistance recipients’ medical card.

FOSTER CARE – Mainly court-ordered residential care for children outside their own homes with families who take up to 4 children; homes are licensed by county social services departments, licensed private child-placing agencies, or by DHS. Regulated under chs. HSS 56 and HFS 38 rules.

FOSTER GRANDPARENTS – Older adults employed by the Department in half-time substitute grandparent capacities at residential institutions for children and youth.

FPI – Family Partnership Initiative; a contracted service which provides wrap-around services for your at risk of out-of-home placement.

FPL – Federal poverty level; Poverty income thresholds by family size; originally developed by the Social Security Administration in 1964; annually updated by the Census Bureau for inflation, with a version simplified for administrative purposes published each February as “poverty income guidelines” by the U.S. Department of Health and Human Services in the Federal Register.

FS – Food Share; formerly known as Food Stamps.

FSA – Family Support Administration; part of Administration for Children and Families in HHS.

FSET – Food Share Employment and Training.

FSH – Food Share Handbook.

FSP – Family Support; A DHS program under s. 46.985, Stats., and ch. HFS 65 rules that provides support to families with a severely handicapped child living at home by helping pay for the services and goods that a family needs to care for the family member at home.

FTP – Family Training Program; a contracted service to teach parenting.

G

GA or GR – General Relief Assistance; a program operated by a handful of counties in WI to provide basic needs and/or medical services for very low-income individuals who do not qualify for other public assistance programs.

GPR – General Purpose Revenues; consist of state-levied taxes and miscellaneous receipts and revenues collected by state agencies and paid into the state general fund where they are then available for appropriation by the Legislature.

GPR EARNINGS – Those dollars collected by the Department in payment for its services but deposited in the general fund rather than kept by the Department or earmarked for expenditure by it.

GRIEVANCE – An expression of dissatisfaction about any matter other than an action.

GROUP HOME – Residential care, mainly court-ordered, for children outside their own homes with families or organizations that take from 5 to 8 children; homes are licensed by the Department and regulated under ch. HSS 57 rules.

HALFWAY HOUSE – A short-term structured living environment mainly for persons who have just left residential institutions and are expected to have difficulty adjusting to living in the community.

HANDICAPPED-ACCESSIBLE – A term applied to buildings, rooms and restrooms that are accessible to persons with disabilities. A room or building must meet specific DHS standards to meet the definition of handicapped-accessible.

HCFA – Health Care Financing Administration in HHS.

HCT – Home and Community Treatment Program; at Mendota Mental Health Institute; demonstrates a community-based model for providing mental health treatment for pre-adolescent children and their families without hospitalization.

HEALTHCHECK – Wisconsin term for EPSDT.

HEALTHY BIRTH PROGRAM – Department program dating from 1985 and directed toward reducing the incidence of low birth weight and birth defects and ensuring that all infants receive comprehensive health care throughout their first year of life.

HEALTHY START PROGRAM – Department program dating from 1988 providing Medical Assistance benefits for pregnant women and children to age 6, with family incomes up to 155% of federal poverty guidelines. Now referred to as Badger Care Plus.

HFS 1 – DHS administrative rules for the Uniform Fee System.

HFS 34 – DHS administrative rules for emergency mental health service programs.

HFS 35 – Outpatient mental health services.

HIPAA – Health Insurance Portability and Accountability Act; Common name for Public Law 104-191 containing government mandates for the healthcare industry, including EDI administrative simplification requirements.

HIRSP – Health Insurance Risk Sharing Plan; under ch. 149, Wisconsin Statutes, and ch. HFS 119 rules. Responsibility transferred from OCI to DHFS by 1997 Wisconsin Act 27. A health plan for high-risk individuals who might not be able to obtain any other health insurance.

HIV – Human Immunodeficiency Virus; the virus that causes AIDS.

HLAA – Hmong-Lao American Association.

HOME CONSULTANT – DHS staff who assist eligible individuals in their home and community by providing Supportive Home Care and Daily Living Skills training.

HOSPICE – A residential or nonresidential program that provides care to persons who are dying, with the care provided not being directed at a cure of the individual but rather at lessening pain and other physical symptoms, with the provision of various kinds to support to the individual and the individual’s family. Hospices are licensed by DHFS under ch. HSS 131 rules.

HSD – County Human Services Department; established under s. 46.23, Wisconsin Statutes, through a merger of the county social services department, the county department of community programs established under s.51.42, Wisconsin Statutes, and other county human services agencies.

I

I & A – Information and Assistance; Service provided by the Resource Centers using a telephone number that is toll-free to all callers in its service area. Information provided is related to aging, physical and developmental disabilities, chronic illness and long-term care, including referrals to and assistance in accessing services.

I & R – Information and referral; a listing of services and agencies on the Oshkosh Public Library database.

IADL – Instrumental Activities of Daily Living; refers to tasks required to maintain an independent household. Activities include such tasks as meal preparation, light housework, using the telephone, arranging and using transportation and the ability to be functional at a job site.

ICD-9-CM – International Classification of Diseases; 9th edition, Clinical Modification.

ICF – Intermediate care facility; a federal Title XIX term for MA reimbursement purposes to a lower level of nursing care than that provided in a skilled nursing facility (SNF).

ICF-MR – An ICF serving only or mainly mentally retarded residents, providing active treatment for residents, and certified under 42 CFR 435 and 442. In Wisconsin these are called facilities for the developmentally disabled (FDDs).

ICPC – Interstate Compact; governing agreement applicable when children move between states.

ICU – Intensive Care Unit; in a hospital.

IDA – Individual Development Accounts

IDT – Interdisciplinary Team; In Family Care, at a minimum includes, case manager, a registered nurse and consumer.

IEP – Individual Education Plan; a plan developed by schools for special education students. It identifies the child’s needs and the steps that will be taken to meet the needs.

INDIRECT SERVICES – Services to people provided by the Department through various public and private agencies under aid formulas, purchase agreements or contracts.

INTERDISCIPLINARY TEAM (Care Team) – Referred to as your ‘care team’ in this booklet. The interdisciplinary team, which includes the member, defines the member’s personal outcomes and creates the individual service plan.

IRB – Institutional Review Board; for protection of human subjects of research. Required by 42 CFR 46 in every institution and agency conducting research involving human subjects with HHS funds.

IRIS - Include, Respect, I Self-Direct; a Wisconsin statewide, self-directed, home and community waiver program.

ISP – Individual Service Plan; Upon enrollment, the care team shall develop and implement an initial ISP, based on information received from the ADRC and on the MCO’s initial assessment of the member’s needs. The initial ISP shall be developed by the MCO in conjunction with the member. The MCO shall contact the member within three calendar days of enrollment to develop an initial ISP and the initial ISP shall be signed by the member within ten (10) days of enrollment. The ISP is a supplement of the Member-Center Plan.

ITF – Inpatient Treatment Facility

IV-E – A complex funding formula established by the Feds to assist counties.

KATIE BECKETT PROGRAM – A Medical Assistance program (also called SSI Deeming) for disabled children who need skilled or intermediate nursing care, with only the child’s income and assets considered and not the income and assets of the parents.

KIDS – Kids Information Data System; An automated system for the DWD and county child support enforcement program.

KINSHIP CARE – Program under s. 48.57 (3m), Stats., to provide monthly payments of $215 to support a child who is being provided care and maintenance by a close relative, called a kinship care relative. DHS or the county social services or human services department approves the payment if it determines that the child needs to be placed with the relative, the placement is in the best interests of the child, and the relative meets specified conditions.

LIVING HEALTHY PROGRAM – Oshkosh-based community clinic for low-income people who do not qualify for Medicaid; funded in part by a grant from the Winnebago County Department of Human Services

LOC – Level of Care; The level at which an individual screens functionally eligible for Family Care, either comprehensive or intermediate.

LPHA – Local Public Health Agency

LSS – Lutheran Social Services of Wisconsin and Upper Michigan; a licensed child-placing agency.

LTC – Long-Term Care; A range of services that addresses the health, personal care, and social needs of individuals who lack some capacity for self-care. Services may be continuous or intermittent but are delivered for sustained periods to individuals who have a demonstrated need, usually measured by some index of functional incapacity.

LTCFS – Long Term Care Functional Screen; a screening process used by the Division of Long Term Support to determine eligibility for COP and Waiver funding.

LTE – Limited Term Employee

MA – Medical Assistance; (Medicaid) Title XIX of Social Security Act.

MAPT – Medicaid Administration Pass-Through; An administrative program to arrange for the pass-through to counties of the federal share (50%) of certain administrative and quality assurance functions reimbursable through Medicaid administrative funds. Currently this funding is available to counties to develop community living options for people who are elderly and people who have a disability. The availability of a 50% federal match on Adult Family Home development activities will assist counties in creating additional community living capacity.

MA WAIVER – An exemption from a requirement of federal Medical Assistance (Medicaid) law, granted to the Department by the Secretary of the U.S. Department of Health and Human Services under the authority of 42 USC 1396n (b), (c) or (d). Currently, the Department has a waiver under which it requires MA recipients in Milwaukee County to participate in HMOs that are under contract to the Department for this purpose, and waivers that enable the Department to provide MA-funded home and community-based services under CIP I-A, CIP1-B, CIP II and COP to MA-eligible elderly and disabled persons who would otherwise need SNF or ICF level of care in nursing homes.

MAAC – Medical Assistance Advisory Committee; advises the Division of Health about administration of the MA program.

MAPP – Medicaid Purchase Plan

MCH – Maternal and Child health (Title V) program of the Department.

MCO – Managed Care Organization; Operates Family Care and Partnership long-term care programs.

MCP – Member-Centered Plan; The plan developed by the CMO staff and the Family Care member which outlines the member’s preferences and personal outcomes. The plan should inform the Individualized Service Plan (ISP) which records services and supports needed in order to meet the Family Care member’s outcomes.

MEDICAID – Title XIX of the federal Social Security Act and 42 CFR 430 to 456; pays for medical care for low-income persons; state-administered; same as Medical Assistance.

MEDICAL ASSISTANCE (MA) – Wisconsin’s term for the Medicaid (Title XIX) program which pays for necessary health care services for persons whose financial resources fall within the guidelines.

MEDICARE – Title XVIII of the federal Social Security Act and 42 CFR 405 to 424; insurance-like payments for medical care of persons aged 65 and over; administered by federal Social Security Administration.

MEMBER – A person who is enrolled in Family Care of Partnership.

MEMBER-CENTERED PLAN OR MCP – An ongoing plan that documents a process by which the member and the interdisciplinary team (IDT) further identify, define, and prioritize the member’s personal and quality of life outcomes. The MCP includes how informal and community resources, and services and supports available through the MCO benefit will be used to achieve the member’s personal outcomes. The ISP is a part of the MCP. The MCP shall be completed within 60 calendar days from enrollment.

OPRA – DHS Office of Program Review and Audit

OPTIONS COUNSELING – RCs offer consultation and advice about the options available to meet an individual’s long-term care needs. This consultation will include discussion of the factors to consider when making long-term care decisions. Resource centers will offer pre-admission consultation to all individuals with long-term care needs entering nursing facilities, community-based residential facilities, adult family homes and residential care apartment complexes to provide objective information about the cost-effective options available to them. This service is also available to other people with long-term care needs who request it.

PAC – Pre-Admission Consultation; Consultations designed to inform individuals of available long-term care options and counsel them regarding their options before making permanent decisions on their LTC. It is also an opportunity to determine if they are eligible for family care.

PACE – Partners in Alternative Care Education; the agency administered training program required of foster parents for children.

PACE – Program of all-inclusive Care for the Elderly. Operated in Milwaukee by Community Care for the Elderly and in Dane County by Eldercare, under contract with DHFS.

PACT – Program for Assertive Community Treatment; an outpatient program for adults operated by Mendota Mental Health Institute.

PART H – Early intervention program for infants and toddlers (age 0-2) with disabilities under PL 99-457. This is now Part C of PL 105-17. See BIRTH TO 3.

PD – As a noun physical disability or disabilities; as an adjective or adverb, physically disabled.

PEP – Performance Enhancement Plan; the State plan to meet the federal standards prior to the next Child Welfare audit.

PERMANENCY PLANNING – Planning and services by a social service agency to reunite a child in substitute care with his or her natural parents. When this is not possible or not in the child’s interest, the agency seeks termination of parental rights and arranges for the child’s adoption or attempts to locate and support another permanent placement for the child. The goal is that every child for whom the social service agency is responsible grows up in one permanent home. See s. 48.38, Wisconsin Statutes.

PERSONAL CARE SERVICES – Mainly refers to assistance with activities of daily living such as eating, dressing, bathing and walking.

PERSONAL OUTCOMES – Represent what is important to the member, or are things the member wishes were different in his or her life.

PHI – Protected Health Information

PL – Public Law; Federal term equivalent to session law in Wisconsin.

PL 94-142 – The Education for All Handicapped Children Act; provides basic requirements for the educational system in serving handicapped children.

POS – Purchase of services

POVERTY LEVEL – See FEDERAL POVERTY LEVEL.

POWER OF ATTORNEY FOR HEALTH CARE – Under ch. 155, Wisconsin Statutes, a competent person at least 18 years of age may complete a form available from DHFS to designate someone to make health care decisions for the person in the event that the person becomes unable to make those decisions. The designated person is that person’s agent to exercise the power of attorney.

POWER OF ATTORNEY FOR PROPERTY AND FINANCE – Under ch. 243, Wisconsin Statutes, competent person must be at least 18 years of age to designate someone to make property and finance decisions for the person in the event that the person cannot.

PREFERRED PROVIDER ORGANIZATION (PPO) – A limited or preferred group of physicians and hospitals (providers) that offer a discount on their normal charges for services provided to members of an organization which contracts with it

PROGRAM ACCESSIBILITY – Federal/state requirement that each program or activity funded through DHFS is operated so that the program or activity, when viewed in its entirety is readily accessible to and usable by persons with disabilities.

PROTECTIVE PLACEMENT – Under s. 55.06, Wisconsin Statutes, court-ordered placement of persons found incompetent to manage their own affairs and in need of care and custody; placement is in nursing homes and other non-psychiatric protective living settings.

PROTECTIVE SERVICES – Assistance to protect vulnerable persons from being abused, exploited, or treated in a degrading manner, and to help those who are adults live as independently as possible.

PROVIDED SERVICES – Services for people actually provided by staff of the Department of local human service agencies, rather than purchased.

PS – Protective Services

PST – Placement Screening Team; a meeting attended by youth, their family, social worker, their supervisor, and the Resource Team supervisor to address service needs and potential placement needs of a youth.

PT – Physical Therapy

RAD – Resource Allocation Decision; A step-by-step tool you and your care team will use to help determine the most effective and cost-effective way to achieve your outcomes.

RC – Resource Center; Entity offering a variety of services, including one-stop shopping for older adults, people with disabilities, and their family members for a wide range of information and providers that are available in the local communities. The RCs also provide counseling about long-term care options and eligibility determination for the Family Care benefit and serve as a clearing-house of information designed to assist service personnel working with populations in need of long-term care services.

RCAC – Residential Care Apartment Complex; name for Assisted Living Facility. See Assisted Living Facility.

RCC – Residential Care Center; a privately operated residential facility licensed by the Department to provide residential care and treatment for more than 8 youths with mental, emotional or behavioral problems; there are 36 RCC’s in Wisconsin. Regulated under ch. HSS 52 rules.

REHABILITATION ACT OF 1973 – Federal vocational rehabilitation and independent living services legislation, 29 USC 701-796, which governs state administration of employment and independent living services for people with disabilities.

RESPITE CARE – Temporary, intermittent relief for the family member or other person taking care of an elderly or disabled adult or a child at risk.

RFP – Request for proposal; a document that solicits proposals from outside parties in a competitive bidding process.

RMS – Random Moment Survey; the State’s new methodology for determining allocations for W-2 and Income Maintenance programs.

ROLO – Wisconsin Supreme Court Decision in which Mr. Rolo was the defendant.

SED – Severely Emotionally Disturbed

SENIOR CARE – Prescription program for low-income elderly; began September 2002.

SHELTERED EMPLOYMENT – Facility based supervised work for the disabled.

SHOCAP – Serious Habitual Offender Comprehensive Action Program; SHOCAP is a comprehensive and cooperative information and case management process for police, prosecutors, schools, human services, corrections, and social and community after-care services regarding habitual juvenile offenders.

SINGLE AUDIT – A comprehensive financial and compliance audit pursuant to OMB Circular A-128 of a county receiving federal funds through the Department; reviewed by Department audit staff.

SJO – Serious Juvenile Offender; youth adjudicated by the court to be SJO. They must have committed specified crimes. The state assumes custody of them for up to five years.

SNF – Skilled nursing facility; a federal Titles XVIII and XIX certification term and state licensing term for long-term care facilities that provide care to residents who no longer need the type of care and treatment provided in a hospital but do require some medical attention and continuous skilled nursing observation.

SOFSP – State Option Food Share Program

SOGS – Southern Oaks Girls School; correctional placement for girls.

SPC – Specific Program Category

SPRITE – Support Perseverance Respect Initiative Teamwork and Education; a correctional program which provides an adventure-based education program designed to teach delinquent youth to become re-integrated into their community.

SPOUSAL IMPOVERISHMENT LAW – Refers to 49.455, Wisconsin Statutes, which is designed to prevent the impoverishment of the non-institutionalized spouse of an institutionalized MA recipient by reserving part of the couple’s income and resources for the support of the spouse who is not institutionalized.

SSBG – Social Services Block Grant; A federal grant which provides funds to states for a variety of social services, and which in turn is distributed by the Department on a formula basis to counties which provide or purchase these services for people who need them.

SSTOP – Safe Streets Treatment Options Program; A program for second and third time OWI offenders.

STATEWIDE BED LIMIT – The maximum number of nursing home and FDD beds that may be licensed in the state. Established biennially by the Legislature under s. 150.31, Stats,. With adjustments made by the Department under ch. HSS 122 rules.

SUD/IDP – Substance Use Disorder/Intoxicated Driver Program

SUBSTANCE USE DISORDER 

SUM CERTAIN – An appropriation providing funding up to but not exceeding a certain amount.

SUM SUFFICIENT – An appropriation providing whatever level of funding is necessary to accomplish the purpose for which it was made.

SUPERRULE – Chapters HFS 101 to 108, Wis. Adm. Code, the Department’s rules for administration of the Medical Assistance (MA) program.

SUPPORTED EMPLOYMENT (SE) – Non facility or community based supervised work for the disabled.

SUPPORTIVE HOME CARE – Care provided to elderly and disabled persons residing in their own homes; consists of assistance with daily living needs, including household care and personal care.

UNIVERSAL PRECAUTIONS – Precautionary measures to be taken by health care workers in regard to blood and other body fluids when caring for patients, as recommended by the U.S. Public Health Service’s Centers for Disease Control (CDC) to prevent transmission of HIV and other blood-borne and body fluid-borne infections. WISCONSIN FAIR EMPLOYMENT ACT – Sections 111.31 to 111.395, Stats. Prohibits discrimination in employment based on age, race, creed, color, handicap, marital status, sex, national origin, ancestry, sexual orientation, arrest record or conviction record.

Wisconsin works (W-2) – Wisconsin’s TANF program which began in 1997 under the state Department of Workforce Development.

WMHI – Winnebago Mental Health Institute; near Oshkosh.

WPL – Wisconsin Power & Light

WRA – Wisconsin Rehabilitation Association

WRC – Wisconsin Resource Center; near Oshkosh administered by the DHFS Division of Care and Treatment Facilities for the purpose of providing individualized care and treatment for mental illness, a developmental disability or substance abuse to inmates transferred from the Department of Correction’s maximum and medium security institutions, and to provide treatment to persons committed to the Department under ch. 980, Stats.

WSSA – Wisconsin Social Services Association

WtW – Welfare to Work Program

W-2 – Wisconsin Works; the state’s program to replace AFDC. Makes cash assistance dependent on taking a job or undertaking job training. Cash benefits limited to 60 months. Administered by DWD. Began September 1997.

YCSP – Youth Corrective Sanctions Program; a State administered electronic monitoring plan for youth with orders to corrections.

YOUTH AIDS – Funds distributed each year by DOC to county social service and human service departments to pay for state correctional institution services for youths found by courts to be delinquent, and to pay for development of alternative community programs and services. These include residential care, mainly for youths alleged to be or found to be delinquent and for youths alleged to be or found to be in need of protection and services because of behavior which would not be a crime if committed by an adult.
