Community Support Program is a state-certified coordinated care and treatment program under HSS 63 for persons diagnosed with a severe and persistent mental illness. A range of treatment, rehabilitation and support services are provided through a multi-disciplinary team of staff. Winnebago County has two CSP Programs, one in Neenah and one in Oshkosh.
The purpose is to provide comprehensive community-based care in the least restrictive environment consistent with individualized needs and incorporate the concept of recovery.

Community Based Care - MH
Community-based, side-by-side contacts with the person to provide concrete direction and assistance in acquiring and maintaining needed life resources.
Medication Monitoring
Monitoring a person's mental status and response to medication prescribed by a psychiatrist. Coordinating with the psychiatrist regarding pharmacological treatment. Sometimes involves observing consumers taking medication.
Psychiatric Evaluation - MH
Upon referral from any other Behavioral Health treatment team, staff psychiatrists perform diagnostic evaluations for the purpose of initiating adjunctive medication prescription services for the referred consumer. Psychiatric evaluations are also sometimes performed for the purpose of determining a consumer's need for involuntary treatment or protective services.
Supportive Counseling
Supportive and educational counseling about a person's mental disorder and life problems, and problem-solving with the person regarding the same.
Vocational Services
Services are delivered for the purpose of promotoing vocational participation and self-sufficiency. Services may be delivered in either community jobs or in sheltered work settings. Employment services may include, but are not limited to, vocational assessment, planning, and job development and/or coaching on the job.
